Overview
“Multimorbidity Wheel” aims to help medical professionals with an overall picture of their patients health status. It fuses all of the patient’s diagnoses into a single nested doughnut chart, which always has the same shape, aiding in recognizing common patterns in patient groups and helping to tailor the care to these disease-complexes.
This is implemented as part of shEHRd, my online health record, and JanKIS, my health information system for hospitals, primary care, laboratories, pharmacy and public health agencies.
